NAssess is a C# class library that makes putting surveys on the web (and other media) very easy. Its primary targets are PDFs and ASP.NET. It's been in private development for 5 years before it was open-sourced.

NAssess is unique among online survey engines in that it targets research surveys instead of marketing surveys. Its native file formats are built around the way researchers work.

Its most unique trait is that NAssess produces reports for the research subjects rather than the data collector. This becomes useful if the researcher wants to give back to the community by providing each of his/her research subjects with an assessment of how he/she is doing based on their answers to his questionnaire.
